What administrative documentation is required to process an Access 3 Order?

A fully executed System Information Document must accompany the initial order. This is MANDATORY for all Access 3 orders. Orders can not be entered without this document.

On subsequent orders for AP:

• A letter of authorization will be required if the product is shipped to an address other than what is recorded on the System Administration Document, and/or if the address on the purchase order is different than what is listed on the System Administration Document.

On subsequent orders for AS and AHS:

• A specific randomized security code will be required for each order unless "Opt-Out" has been authorized by the system administrator. Refer to the System Information Document for "Opt-Out" details and instructions.
